Travel Physical Therapist (PT) - $2,733 per week in Newton Center, MA

Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8.5 Experience: Minimum 2 years of home care experience License: Physical Therapist Professional license in Massachusetts Certifications: Certified Home Health Care experience required Must-Have: 1 year of case management experience Description: Provide physical therapy services to patients in their place of residence. Assist patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals. Identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation. Engage in interdisciplinary communication in accordance with agency policies and standards of practice. Weekend rotations are required every 6 weeks. Ability to travel to assigned cases is necessary. Experience in hospice, private care, or in certain case management roles does not meet the qualifications for certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the experience requirement.
