The role focuses on providing exceptional home health services, facilitating comprehensive evaluations, and devising personalized treatment plans to help patients achieve independence and improved function. Candidates will operate across Washington, Scott, and parts of Floyd and Clark counties.

Key Responsibilities of the Physical Therapist

Perform physical therapy evaluation and treatment based on physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Review patient records for diagnosis, medical history, and precautions
Establish goals and develop treatment plans for patient independence and function
Regularly reassess treatment effectiveness and patient progress
Communicate patient needs and progress to the treatment team and family
Consult with medical providers on treatment and discharge plans
Recommend adaptive equipment to enhance rehabilitation
Maintain and submit documentation as required by regulations
Adhere to professional practice standards and state/federal laws
Supervise PT Assistants and Aides in patient care activities
Participate in staff development programs and continue education

Requirements of the Physical Therapist

Indiana License
Home health experience
Proficiency with HCHB and OASIS
At least two years of recent experience
