Key Responsibilities of the Physical Therapist - Travel- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop treatment plans
- Regularly re-assess treatment plans and patient progress
- Communicate patient needs and progress with the treatment team and family
- Consult with medical providers regarding patient care and progress
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit required documentation as per regulations
- Supervise Physical Therapist Assistants and Aides in patient care
- Participate in and/or conduct professional development and meetings
Requirements of the Physical Therapist - Travel- KY Physical Therapist Licensure or Compact Licensure
- 1 year home health physical therapy experience
- Previous OASIS experience
- Home Care Home Base experience
