Key Responsibilities of the Physical Therapist

Perform physical therapy evaluation and treatment based on physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Establish measurable goals and develop treatment plans
Re-assess effectiveness of treatment plans and modify as needed
Communicate patient needs and progress to the treatment team and family members
Consult with medical providers regarding patient evaluations and progress
Recommend and assist in securing adaptive equipment for patients
Maintain and submit documentation as required
Supervise Physical Therapist Assistants and Aides
Participate in staff development programs and obtain continuing education

Requirements of the Physical Therapist

License as a Physical Therapist
Home Health experience required
HCHB experience preferred but not required
