Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active California Physical Therapy license required to start the assignment.
  • Minimum 1 year of recent skilled home health experience, with at least 1 year within the last 3 years.
  • OASIS experience required for clinicians performing start of care visits.
  • HCHB electronic documentation experience required.
  • BCLS (BLS) certification required.
  • Traveler must have reliable personal transportation. Occasional travel radius may be up to 50+ miles.
  • Experience treating a geriatric caseload preferred.


Assignment Details

  • Facility Type: Home Health
  • Shift: Days
  • Shift Days: Monday through Friday
  • Shift Hours: 8:00 AM to 5:00 PM
  • Case Load: Geriatric, typical ratio 1:1; may visit up to 6 patients per day. Standard productivity expectation: 30 points.
  • Documentation System: HCHB
  • OASIS required for start-of-care visits.
  • Traveler is expected to provide their own transportation. System groups clients to minimize drive time.
