Travel Physical Therapist (PT) - $2,758 to $2,958 per week in Eureka, CA

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CA license required
• BCLS required
• HCHB required
• 1 year of recent Skilled Home Health experience within the last 3 years required
• OASIS experience required
• Own transportation required

Assignment Details

• Monday through Friday, 8:00 AM to 5:00 PM
• Geriatric caseload
• May visit up to 6 patients per day
• 30 productivity points standard
• Occasional travel radius of 50+ miles
• 1:1 patient ratio
