Travel Physical Therapist (PT) - $2,759 per week in Portland, OR

Job Title: Physical Therapist (PT) Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Conduct home visits for therapy services. Attend periodic office meetings. Must have reliable transportation. A current driver’s license is required. Agency-required liability insurance is needed. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. These assignments equate to roughly 6-8 productivity points each day. Guaranteed hours are not available. If a low patient census occurs, please discuss options for floating to nearby clinics for hours. Types of cases include Start of Care and Follow Ups. First-timers are not accepted; recent Home Health experience is required. Weekend requirements vary per location and profession. On-call requirements vary by job/location; please discuss during interviews. Must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. Travel compensation is provided from patient home to patient home. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted unless the license can be obtained in a few days. Most assignments will be standard 5x8 schedules Monday through Friday. There will be no weekend rotation and no on-call requirements. This position falls under home health services. Referral demand has created the need for additional contractors, alongside new hires in onboarding/training stages. Documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in a cancellation. There are no guaranteed hours with this contract. Weekly productivity expectations are set at 30 points, which equate to 40 hours.
