Physical Therapist - Home Health - (PT - HH)

Pay Rate: $2762.00 - $2846.00
Physical Therapist - Home Health jobs in Keene, NH offer Monday through Friday 8-hour days, covering 40 hours per week. You will visit 6 patients per day, with a total caseload of 30 patients, using Homecare Homebase EMR.
Your responsibilities include evaluating and treating patients in their homes, developing care plans, and collaborating with interdisciplinary teams. You will document progress and help patients regain mobility and independence. Home health experience is preferred, but if you do not have it, you must commit to a 26-week assignment. A current NH license is required.
