Job Title: Physical Therapist Profession: Physical Therapist Specialty: Physical Therapy Duration: 13 weeks Shift: Day Shift Hours per Shift: 5 x 8s, M - F with an every 3rd or 4th weekend Experience: Minimum of 2 years of Physical Therapy experience preferred License: Physical Therapy licensure in state of practice required Certifications: Basic Life Support (BLS) certification from an approved vendor required Must-Have: Experience in working with an adult and geriatric population required Description: Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions. Serves as co-leader in interdisciplinary approach to patient groups. Completes evaluations to ensure quality standards and addresses areas that require attention. Uses appropriate clinical techniques and tools to establish individualized treatment plans for patients. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures communication to appropriate parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned. Commits to the safety and well-being of associates and patients. Requires that all associates receive all required vaccinations as a condition of employment annually.
