Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Londonderry, NH offer 13-week contracts with Monday through Friday 8-hour day shifts and 40 hours per week. You must have a New Hampshire PT license, and home health experience is preferred. The coverage area includes Salem, Nashua, and Manchester, with an average of 6 visits per day. You will use Homecare Homebase EMR and receive mileage reimbursement at $0.72 per mile.
Your responsibilities include performing diagnostic tests and measurements, developing and implementing individualized care plans, providing manual therapeutic exercises, gait training, and balance interventions. You will educate patients and families on home exercise programs and document all care accurately[1].
