Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Southboro, MA are available for 13-week assignments with Monday through Friday 8-hour days. You must have an active Massachusetts license and home health experience. You will complete 6 to 7 patient visits per day, with visits scheduled in a tight radius covering Framingham, Natick, Hopkinton, and Westboro. This role uses the Homecare Homebase EMR system. Mileage is reimbursed at $0.72 per mile. Your responsibilities include evaluating patient needs, developing and implementing individualized treatment plans, documenting progress in Homecare Homebase, and collaborating with healthcare teams to support patient mobility and independence 1.
