Job Title: Physical Therapist Profession: Physical Therapist Specialty: Physical Therapist Duration: 8 weeks Shift: Day Shift Hours per Shift: 8 hours per day, 5 days a week Experience: Minimum of two years of Physical Therapy experience preferred License: Physical Therapy licensure in state of practice required Certifications: Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required Must-Have: Recent EPIC experience Description: Focused on growth and technological expertise since 1974, this facility is a full-service community and regional teaching hospital. We are comprised of a 156-bed medical center serving multiple rural cities with primary and specialty care services. This community offers a nice balance of small-town atmosphere and cultural diversity. Job Summary: Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions. Meets department objectives while serving as a co-leader in an interdisciplinary approach to patient groups. Job Requirements: Education: Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016; or foreign equivalent. Two years of Physical Therapy experience is preferred. Licenses/Certifications: Physical Therapy licensure in state of practice is required. CPR or BLS certification from an approved vendor is required. Essential Functions: Completes evaluations to ensure quality standards and addresses assessment areas requiring attention. Uses appropriate clinical techniques and tools to establish individualized treatment plans for patients. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans, ensuring communication to appropriate parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned. Organizational Requirements: This organization is committed to the safety and well-being of all associates and patients.
