Key Responsibilities
- Evaluate patient functional status and develop individualized treatment plans
- Deliver skilled physical therapy including therapeutic exercise, manual therapy, and neuromuscular re-education
- Document evaluations, progress notes, and discharge summaries per facility protocols
- Supervise and delegate appropriate tasks to PTA and rehab aide staff
- Participate in interdisciplinary care conferences and family education
- Maintain productivity standards and billing compliance
Requirements
- Active state PT license (or eligibility for endorsement)
- CPR/BLS certification current
- Minimum 1 2 years of clinical PT experience; SNF/LTC or post-acute setting preferred
- Strong documentation skills and productivity management
- Ability to evaluate, plan, and supervise caseloads across multiple payer types
- Active New Mexico Caregivers Registry listing required for placement at NM facilities
