Travel Physical Therapist - Home Health job in Concord, NH offers you a rewarding opportunity to provide care across the Lebanon, NH area. You will work Monday through Friday, 8-hour days, with 40 hours expected each week. Your responsibilities include completing 6 home health visits per day, managing a caseload of 30 patients, and documenting in Homecare Homebase EMR. NH license is required, and home health experience is preferred; if you do not have home health experience, you must commit to a 26-week assignment.
