90% OP, 10% IP. General Ortho, Neuro, Gait. 20 bed Critical access hospital. -Will accept IL Lic Waiver OSF ITEMS OF NOTE: - Local Rate: Applies to clinicians with permanent addresses within 50 miles (-# to regular and OT bill rates) - OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift - Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation REQUIREMENTS FOR SUBMISSION: - Last four (4) digits of clinician's SSN - Clinician's middle initial - Requested time off - Active IL license or confirmation that application process has been initiated - Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels POSITION SUMMARY: The Physical Therapist assists patients in the restoration of physical function through evaluation and treatment. REQUIRED QUALIFICATIONS: Education: Doctoral degree in Physical Therapy *Those licensed after 2016 must have a Doctoral degree. Those who were licensed prior to 2016 are exempt from this requirement as grandfathered under prior IDFPR or MBPT requirement for licensure (which allowed bachelor s/master s degree; the normal industry requirement prior to 2016). Licensure/ Certification: Respective state license in Physical Therapy or has graduated successfully from a professional program, is in receipt of Authorization to Test Letter, is scheduled to test within 60 days of eligibility, and has not failed the licensure exam. After passing licensure exam, state license must be verifiable within one year of hire. CPR certification within 90 days of hire Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Experience: Previous experience as a Physical Therapist Licensure/ Certification: Valid state driver s license, as verified through the respective state Department of Motor vehicles driving record abstract. Other Skills/ Knowledge: Demonstrate the knowledge and skills necessary to provide patient care appropriate to the population served. Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information. Analytical ability to evaluate patients' physical and functional status, establish therapy goals, formulate treatment plans consistent with goals, observe patients and document status, and assist techs and assistants in resolving problems.
