Travel Physical Therapist (PT) - $2,859 per week in Newton Center, MA

Description: As a Home Health Therapist, you will be responsible for assessing patient conditions. You will implement care plans tailored to the individual needs of your patients. Ongoing education and support will be provided to patients and their families. Patient care must be delivered in accordance with organizational policies and standards. You will be required to perform admission processes and case management responsibilities. Documentation and communications must be thorough and timely.
