Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals.
• Complete a comprehensive evaluation using appropriate assessment tools and procedures.
• Review patient records for current diagnosis, medical history, and any precautions.
• Establish measurable goals and develop a treatment plan to maximize patient independence and function.
• Regularly re-assess treatment effectiveness and patient progress towards goals.
• Communicate patient needs and progress to the treatment team and involved parties.
• Consult with medical providers regarding patient evaluation, treatment, and discharge.
• Recommend and assist in securing adaptive equipment for patients.
• Maintain and submit required documentation in accordance with regulations.
• Supervise Physical Therapist Assistants and Aides as necessary.
• Participate in internal staff development programs and continue education as required.

Requirements of the Physical Therapist

• PT License
• Experience with HCHB and OASIS
