Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician orders/referrals
  • Conduct comprehensive evaluations using appropriate tools and procedures
  • Review patient records for diagnoses, medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans for patient independence and function
  • Re-assess treatment efficacy and patient progress, modifying as needed
  • Communicate patient needs and progress to treatment team, physicians, and families
  • Consult with medical providers regarding evaluations, treatments, and discharges
  • Recommend and secure adaptive equipment to enhance rehabilitation programs
  • Maintain and submit documentation as required by regulations and company policy
  • Supervise Physical Therapist Assistants and Aides
  • Participate in staff development and continue education to enhance skills

Requirements of the Physical Therapist
  • Valid PT license
  • Experience with HCHB and OASIS
  • Productivity requirement of 30 points
