Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis and medical history
• Establish measurable goals and develop treatment plans
• Regularly reassess treatment plan effectiveness and patient progress
• Communicate patient needs and progress to the treatment team and relevant parties
• Consult with medical providers regarding patient care and discharge
• Recommend and assist in securing special adaptive equipment for patients
• Maintain and submit documentation in required formats and timelines
• Supervise Physical Therapist Assistants and Aides in patient care
• Attend and participate in staff development programs and meetings

Requirements of the Physical Therapist

• PT license
• Oasis knowledge (must have)
• Home Health experience (huge plus)
• HCHB software proficiency (must have)
