Key Responsibilities of the Physical Therapist

Perform physical therapy evaluation and treatment based on physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Review patient records for diagnosis, history, precautions, and contraindications
Establish and implement measurable goals and treatment plans
Regularly reassess treatment effectiveness and patient progress
Communicate patient needs and progress with the treatment team and family
Consult with medical providers regarding patient evaluation and progress
Recommend special adaptive equipment and incorporate it into rehab programs
Maintain and submit required documentation accurately and timely
Adhere to professional standards and regulations
Supervise Physical Therapist Assistants and Aides
Participate in staff development and continuing education programs

Requirements of the Physical Therapist

PT AR License
Experience with HCHB, OASIS, and Home Health
