Job Title: Physical Therapist Profession: Physical Therapist Specialty: - Duration: 6 months Shift: M-F Hours per Shift: 8:00 AM - 4:30 PM Experience: Five years of progressively responsible PT experience; Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population License: Valid Physical Therapy License Certifications: BLS Must-Have: Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. Demonstrated ability to prioritize in a fast-paced environment. Ability to work independently with minimal supervision. Description: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants. The role involves working in close coordination with the physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety. The Physical Therapist collaborates with the Occupational Therapist to ensure the efficient operation of the rehabilitation department. This position requires participation in interdisciplinary team meetings to develop participant care plans. The Physical Therapist will conduct Physical Therapy assessments to determine care needs, preferences, and goals of the participants. They will deliver and document Physical Therapy interventions in accordance with physician orders and participant care plans. Interventions include providing active and passive exercise, muscle re-education, gait training, and functional training of Activities of Daily Living (ADLs). Supervision of therapy assistants is a key responsibility, including training, instruction, and guidance needed to implement Functional Maintenance Programs. Training and instructions will also be provided to staff, participants, and caregivers in the use and care of therapeutic equipment. The Physical Therapist must remain alert to input from other team members regarding changes in participants' conditions or functional status. Collaboration with the interdisciplinary team will support smooth care transitions between different settings, including hospitals and home. Participation in end-of-life care coordination and support is required. The Physical Therapist is expected to complete all required documentation in a timely and accurate manner while ensuring confidentiality. Flexibility in performing assignments is essential as business needs evolve. Other duties may be assigned as needed.
