- Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population
- Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs
- Highly motivated and self-directed
- Excellent leadership, organizational, and communication skills
- Ability to work independently with minimal supervision
- Commitment to supporting vulnerable seniors
Description:
The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants.
This includes working closely with the physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety.
The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department.
This position facilitates collaborative relationships across the organization and with external stakeholders.
Key responsibilities include conducting Physical Therapy assessments to determine care needs, preferences, and goals for participants.
The Physical Therapist will deliver and document interventions in accordance with physician orders and care plans.
This may include providing active and passive exercises, muscle re-education, gait training, and functional training for activities of daily living.
The Physical Therapist supervises therapy assistants and provides training, instructions, and guidance.
They will also provide support and training to staff, participants, and caregivers regarding therapeutic equipment.
Staying alert to input from team members and participants is crucial to updating team members regarding any changes in a participant's condition.
Facilitating smooth care transitions between different care settings is a key part of the role.
Participation in enrollment support for prospective participants is also required.
The Physical Therapist will manage direct reports, including training and mentoring team members.
They must complete all necessary documentation in a timely and accurate manner while maintaining confidentiality.
Participation in required quality improvement activities, training, and meetings is expected.
Flexibility in performing assignments is necessary as business needs evolve.
Additional duties may be assigned as needed.
