Job Title: Physical Therapist Profession: Physical Therapist Specialty: N/A Duration: 13 weeks Shift: Monday-Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Five years of progressively responsible Physical Therapy experience License: Valid California Physical Therapy License Certifications: BLS Must-Have: - Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population. - Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. - Highly motivated, self-directed individual able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. - Two years supervisory experience with demonstrated ability to mentor and develop team members. - Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. - Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects. - Ability to work independently with minimal supervision. - Demonstrated ability to prioritize in a fast-paced environment. - Commitment to unlocking the full potential of vulnerable seniors. Description: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants both in the clinic and at home. The role requires close coordination with appropriate medical staff and other interdisciplinary team members to maximize participant independence and safety. The Physical Therapist collaborates with other therapy professionals to efficiently operate the rehabilitation department. They are a member of the management team, facilitating collaborative relationships across the organization and with external stakeholders to promote core values and team culture. Duties include conducting Physical Therapy assessments to determine care needs, preferences, and goals of participants. Participation in interdisciplinary team meetings to develop participant care plans is essential. Delivery and documentation of Physical Therapy interventions in accordance with physician orders and care plans are required. The Physical Therapist provides supervision, training, and guidance to Therapy Assistants and participates in training staff, participants, and caregivers/families in the use and care of therapeutic equipment. Managing equipment maintenance and tracking is also a responsibility. The therapist must remain alert to pertinent input from team members, participants, and caregivers and update the interdisciplinary team of any changes in participants' conditions or functional status. Coordinating care transitions between different settings and supporting participant enrollment in the program are also key functions of this role. They are involved in end-of-life care coordination and support, as well as managing direct reports and completing all required documentation in a timely manner. Maintaining confidentiality of all procedures and information concerning team members and participants is imperative. Participation in quality improvement activities, training, and meetings is expected, along with timely completion of mandated trainings and occupational health screenings as needed. Flexibility in performing assignments as business needs evolve is also important. Other duties may be assigned.
