Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatment based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, and precautions
- Establish measurable goals and a treatment plan for patient independence
- Re-assess patient's progress and modify goals/treatments as necessary
- Communicate patient needs and progress with the treatment team and family
- Consult with medical providers about patient evaluation, treatment, and discharge
- Recommend and assist with obtaining adaptive equipment to improve function
- Maintain and submit documentation as required by regulations
- Adhere to professional practice standards and applicable laws
- Supervise Physical Therapist Assistants and Aides
- Participate in and conduct staff development programs and meetings
Requirements of the Physical Therapist- Possess a valid WV PT license or a compact license
- Experience with Homecare, OASIS, HCHB, and exercise programming
