This position focuses on providing home health physical therapy, with opportunities to cover Boyd, Greenup, Lawrence, Carter, and Elliot counties. Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Establish measurable goals and develop treatment plans
  • Regularly reassess the effectiveness of treatment plans
  • Communicate patient needs and progress with the treatment team
  • Consult with medical providers regarding patient progress and discharge planning
  • Recommend and assist with the procurement of adaptive equipment
  • Maintain and submit required documentation in a timely manner
  • Supervise Physical Therapist Assistants and Aides as per state practice acts
  • Participate in staff development programs and maintain clinical skills

Requirements of the Physical Therapist
  • KY Physical Therapist Licensure; Compact Licensure Accepted
  • 1 year home health physical therapy experience
  • Previous OASIS experience
  • Experience with Home Care Home Base software
