Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
• Complete comprehensive evaluations and assessments using appropriate tools and procedures
• Review patient records for accurate diagnosis and treatment planning
• Establish measurable goals and develop individualized treatment plans
• Regularly reassess patient progress and adjust treatment plans as necessary
• Communicate patients' needs and progress to the treatment team and relevant parties
• Collaborate with medical providers regarding patient management and discharge planning
• Recommend and assist in the acquisition of adaptive equipment for patients
• Maintain and submit necessary documentation as per requirements
• Supervise Physical Therapist Assistants and Aides
• Participate in staff development programs and maintain clinical competencies and certifications

Requirements of the Physical Therapist

• Compact State or Maryland State Licensed Physical Therapist
• Experience with Oasis documentation and Homecare Homebase EMR system
• Strong customer service and time management skills
