Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts. Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress. Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety. Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes. Appropriately apply ICD-10 coding for accurate and compliant documentation. Present in-service programs periodically to agency staff and contribute to staff education initiatives. Actively engage in Quality Assessment and Performance Improvement (QAPI) activities, supporting continuous compliance and clinical excellence. Other Duties: Provide care and services in accordance with infection control, safety, and risk prevention protocols. Accurately document clinical interventions, care coordination, and communication in a timely manner. Participate in interdisciplinary meetings, case reviews, and continuing education opportunities. Maintain compliance with HIPAA and all company privacy and confidentiality standards. Represent the organization positively and professionally in patients' homes and community settings.
