Schedule: Mon – Fri 8 am – 4.30 pm
Location: Creve Coeur, MO
Job Type: Contract
Contract Length: 13 weeks
Pay Range: 50/hr- $56/hr
*Pay offered is based on experience, expertise, credentialing, and education.
Duties: Key Responsibilities:
· Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications
· Provide ongoing therapy visits and reassess patient progress
· Develop and implement individualized treatment plans to improve mobility and function
· Educate patients and caregivers on therapeutic exercises and safety practices
· Maintain accurate and timely clinical documentation
· Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
· Ensure compliance with home health regulations and agency policies
Requirements: - Education: Bachelors
- License(s): PT License
- Certification(s): BLS
- Years of experience: 1 year
- Skills/Specialties/Technology: Home Health
