Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and applicable state standards
Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
Deliver skilled interventions and maintenance therapy programs within state-defined scope of practice
Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes
Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
Required Qualifications
Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation
Current Physical Therapist licensure in the state of New Mexico
Current CPR certification
Basic computer proficiency and willingness to learn EMR systems and OASIS documentation
Reliable transportation, valid driver's license, and auto insurance for home visits
Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies
Preferred Experience / Skills
Minimum two years of clinical experience as a Physical Therapist
Prior home health experience is a plus but not required
Familiarity with ICD-10 coding and QAPI activities supporting clinical compliance and excellence
