Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals aimed at progressing patients toward discharge within an established length of stay.
Collaborate with the interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management.
Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
Adhere to discipline-specific state practice acts and regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements.
Required Qualifications
Active Physical Therapist license in good standing in the state of Virginia (or eligibility to obtain)
Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts
Preferred Experience / Skills
Experience in an inpatient rehabilitation or acute care hospital setting
Familiarity with interdisciplinary team collaboration and discharge planning processes
Experience supervising or mentoring PT students or clinical affiliates
Active involvement in professional development, continuing education, or professional organizations
