Conduct comprehensive patient evaluations (60-minute evals) and deliver follow-up treatment sessions (30-minute intervals), developing and modifying individualized treatment plans in alignment with patient goals and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities within an established length of stay
Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements
Required Qualifications
Active Physical Therapist license in good standing in the state of Colorado
Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in clinical documentation, goal writing, and compliance with state, federal, and intermediary guidelines
Commitment to patient and employee safety standards, including safe patient handling and infection control practices
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Active involvement in continuing education and professional development activities
