Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities within an established length of stay
Delegate and direct discipline-specific therapy services for assigned patients
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and regulatory requirements; actively pursue professional development through continuing education and professional organizations
Required Qualifications
Active and unrestricted Physical Therapist license in good standing in the state of Colorado
Ability to perform 60-minute evaluations and 30-minute follow-up treatment sessions in an outpatient setting
Proficiency in clinical documentation including goal writing, medical necessity letters, and charge entry in compliance with applicable guidelines
Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality of care initiatives
