Home Care Physical Therapist

Key Responsibilities- Assess clients' conditions and develop individualized plans of care to maximize mobility and independence.
- Collaborate with an interdisciplinary team to ensure comprehensive client needs are met.
- Perform standard therapy measurement techniques including range of motion, strength, and balance testing.
- Utilize a variety of therapeutic modalities to maximize care provided to clients in their homes.
- Manage a patient caseload, including OASIS documentation and case management responsibilities.
Required Qualifications- Active Vermont Physical Therapist (PT) licensure.
- Home health experience is required.
- OASIS experience is required, as this role includes case management responsibilities.
- Availability with no requested time off during the contract period.
Preferred Experience / Skills- Experience with McKesson, Netsmart, or Homecare Homebase EMR systems.
- Demonstrated ability to work autonomously in a home-based care setting.
