Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to ensure coordinated and effective patient care
- Educate patients and their families on therapeutic exercises and home safety strategies
Required Qualifications- Active Physical Therapist (PT) license in the state of Virginia or compact licensure eligibility
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with MDS documentation and Medicare compliance in SNF environments
