Key Responsibilities- Complete comprehensive evaluations for assigned outpatient patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up appointments, progressing patients toward discharge within an established length of stay
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations
- Adhere to the State Practice Act and all regulatory requirements; actively pursue professional development through continuing education and professional organization involvement
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Knowledge of and adherence to discipline-specific state practice acts and professional codes of conduct
- Compliance with all state-mandated continuing education requirements
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring Physical Therapy students
- Familiarity with performance improvement methodologies and quality initiatives
