Physical Therapist (PT) - Home Health

Schedule / Job TypeContract | Days | Monday to Friday, 8:00 AM to 5:00 PM | 13-Week Assignment
Key Responsibilities- Conduct Start of Care evaluations and follow-up visits for home health patients within an assigned territory
- Meet daily productivity expectations through a combination of Start of Care and follow-up visits
- Document all patient encounters and clinical notes accurately using HCHB software
- Develop and implement individualized physical therapy treatment plans in the home setting
- Coordinate care with the broader home health team to ensure continuity and quality of patient outcomes
Required Qualifications- Active Physical Therapist (PT) license in the state of California
- Experience in a home health setting
- Proficiency with HCHB (Homecare Homebase) EMR software required
- Ability to manage a patient caseload independently within an assigned geographic territory
- No on-call or weekend availability required
Preferred Experience / Skills- Prior experience managing Start of Care assessments in a home health environment
- Familiarity with productivity-based scheduling and visit management
