Contract | Days | Outpatient Setting | 6-Month Contract
Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute initial evaluations and 30-minute follow-up sessions, progressing patients toward discharge within an established length of stay
Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations
Adhere to discipline-specific State Practice Act and regulatory requirements; maintain professional licensure in good standing and uphold professional codes of conduct
Required Qualifications
Active and unrestricted Physical Therapist licensure in the state of Colorado
Graduate of an accredited Physical Therapy program
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Knowledge of and adherence to state and federal documentation and compliance requirements
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Demonstrated involvement in continuing education and professional development activities
