Key Responsibilities- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and families on therapeutic exercises and home programs
Required Qualifications- Active Physical Therapist license in the state of Minnesota
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a SNF or long-term care setting
Preferred Experience / Skills- Prior experience with geriatric patient populations
- Familiarity with MDS documentation and Medicare guidelines in a SNF environment
