Key Responsibilities- Plan and conduct segments of physical therapy treatment programs designed to restore and improve patients' physical, social, and mental functions
- Meet departmental objectives while delivering high-quality, patient-centered care in a home health setting
- Serve as co-leader in an interdisciplinary approach to patient care and group treatment
- Complete required clinical documentation using HCHB and OASIS platforms accurately and in a timely manner
Required Qualifications- Doctor of Physical Therapy (DPT) degree required if licensure was obtained on or after January 2016, or foreign equivalent
- Active Physical Therapy licensure in all applicable states of practice
- Current CPR or BLS certification from an approved vendor (AHA-BLS or AHA-CPR accepted)
- Valid Driver's License; must be at least 21 years of age
- Minimum of 1 year of home health experience
- Proficiency with HCHB and OASIS documentation systems
Preferred Experience / Skills- Experience working within an interdisciplinary home health care team
- Strong organizational and time management skills suited for an independent, field-based work environment
