Key Responsibilities

• Perform patient evaluations and develop individualized treatment plans appropriate to rehabilitation needs within the care continuum
• Deliver effective treatment techniques in a competent and professional manner
• Educate patients, families, and caregivers on applicable health information, treatment approaches, and prevention strategies
• Initiate and participate in the discharge planning process
• Complete department activities including documentation, billing, and support of department operations

Required Qualifications

• Graduate of an accredited Physical Therapy program
• Licensed Physical Therapist — Ohio state license and/or Multi-State Compact license required
• Basic Life Support (BLS) certification required within 90 days of hire
• Evaluation and treatment skills appropriate to the developmental and chronological age of patients on caseload
• Physically, cognitively, and perceptually able to provide patient treatment and assist with physical mobility of patients and equipment

Preferred Experience / Skills

• Professional initiative, objectivity, and strong time management skills
• Strong interpersonal and communication skills for working with patients, families, and multidisciplinary teams
