Key Responsibilities
Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Ohio
Ability to perform 60-minute evaluations and treatments in an outpatient setting
Proficiency in clinical reasoning, treatment planning, and goal-setting focused on patient progression toward discharge
Knowledge of and adherence to state and federal documentation and compliance standards
Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Demonstrated involvement in continuing education and professional development activities
