    Physical Therapist – Outpatient

    Contract | Days | 13-Week Assignment | Onsite

    Key Responsibilities
    • Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
    • Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Ohio
    • Ability to perform 60-minute evaluations and treatments in an outpatient setting
    • Proficiency in clinical reasoning, treatment planning, and goal-setting focused on patient progression toward discharge
    • Knowledge of and adherence to state and federal documentation and compliance standards
    • Commitment to patient and employee safety practices including safe patient handling and infection control protocols
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Demonstrated involvement in continuing education and professional development activities
