Key Responsibilities

• Conduct comprehensive patient evaluations (60-minute evals) and deliver follow-up treatment sessions (30-minute intervals), developing and modifying individualized treatment plans aligned with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
• Promote patient and employee safety through safe patient handling, infection control awareness, and adherence to all clinic safety policies and procedures

Required Qualifications

• Active Physical Therapist license in good standing (must meet applicable state requirements)
• Ability to perform comprehensive evaluations and develop evidence-based, goal-oriented treatment plans
• Proficiency in clinical documentation, including progress notes, goal writing, and letters of medical necessity
• Commitment to ethical practice in accordance with discipline-specific professional codes of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Familiarity with performance improvement methodologies and quality initiatives
