    Key Responsibilities
    • Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements
    • Promote patient and employee safety through safe patient handling, materials handling, infection control, and adherence to all clinic safety policies and procedures
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Indiana
    • Must be a licensed, experienced Physical Therapist — new graduates are not eligible for this position
    • Ability to complete thorough clinical evaluations and apply clinical reasoning to develop appropriate treatment plans and goals
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
    • Commitment to professional standards and discipline-specific code of conduct
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Active involvement in continuing education and professional development activities
