Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Monitor and document patient progress, adjusting plans of care as clinically appropriate
• Collaborate with interdisciplinary care team members to ensure coordinated patient outcomes
• Educate patients and caregivers on therapeutic exercises and safe functional mobility

Required Qualifications

• Active Physical Therapist (PT) license in the state of Tennessee
• Graduate of an accredited Physical Therapy program
• Previous experience in a skilled nursing or long-term care environment
• Ability to work day shift hours

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with MDS documentation and Medicare Part A/B billing in SNF settings
• Experience with restorative and rehabilitative therapy programs
