Physical Therapist (PT) – Home Health

Schedule / Job TypeContract | Days | Monday through Friday, 8:00 AM – 5:00 PM | No weekends or holidays
Key Responsibilities- Conduct start of care assessments, recertifications, resumption of care, and regular patient visits in a home health setting
- Manage a typical caseload of 5–6 regular visits per day, or 3 additional visits on admission days
- Maintain a daily point target of 6.5 points per day
- Perform occasional safety evaluations or one-time teaching visits in hospice situations as needed
- Document patient care accurately and in compliance with home health standards, including OASIS
- Operate independently in the field while utilizing available clinical support resources
Required Qualifications- Active North Dakota Physical Therapy License
- Current BLS certification
- Valid Driver's License and active Auto Insurance
- Ability to travel to patient homes within the service area
Preferred Experience / Skills- Prior home health experience strongly preferred
- OASIS training and experience preferred
- Comfort working independently in the field with remote clinical support
