Key Responsibilities
Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, use of supportive and assistive devices, and wound care
Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system
Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and discharge planning
Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed
Required Qualifications
Active Massachusetts Physical Therapist state license
Minimum of 1 year of physical therapy experience, or completion of at least a Level II clinical experience (new graduates considered)
Experience in long-term care or rehabilitation settings
Adherence to APTA and Massachusetts state practice acts, code of conduct, and professional ethics
Preferred Experience / Skills
Experience working with Medicare A, Medicare B, and managed care patient populations
Familiarity with electronic documentation systems used in long-term care or rehabilitation environments
Experience conducting evaluations and managing a varied patient caseload
