Key Responsibilities- Provide skilled physical therapy services to geriatric patients in a home health setting
- Complete OASIS assessments including start of care evaluations
- Manage a daily patient caseload of up to 6 patients while meeting productivity standards
- Travel to patient homes within the service area, with occasional visits requiring extended drive time
- Document patient care accurately and in a timely manner using HCHB electronic medical records system
Required Qualifications- Active California Physical Therapist license required
- Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
- Proficiency with HCHB (HomeCare HomeBase) documentation system required
- OASIS experience required, including start of care assessments
- Current Basic Cardiac Life Support (BCLS) certification required
- Must have reliable personal transportation
Preferred Experience / Skills- Experience working with geriatric patient populations in a home health environment
- Familiarity with productivity-based scheduling and point systems in home health settings
