Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across an assigned service territory
• Meet a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day after each patient visit
• Travel throughout the designated service area to conduct patient evaluations and treatments

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Minimum of one (1) year of home health experience required
• Proficiency with HCHB (Homecare Homebase) documentation software required
• Ability to travel throughout a broad geographic service area as needed

Preferred Experience / Skills

• Familiarity with the greater Snohomish County area and surrounding communities
• Strong time management skills with the ability to manage a high-volume caseload efficiently
