Physical Therapist (PT) - Home Health

Key Responsibilities- Provide skilled physical therapy services to patients in a home health setting across an assigned service territory
- Meet a minimum productivity expectation of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day after each patient visit
- Travel throughout the designated service area to conduct patient evaluations and treatments
Required Qualifications- Active Physical Therapist (PT) license in the state of Washington
- Minimum of one (1) year of home health experience required
- Proficiency with HCHB (Homecare Homebase) documentation software required
- Ability to travel throughout a broad geographic service area as needed
Preferred Experience / Skills- Familiarity with the greater Snohomish County area and surrounding communities
- Strong time management skills with the ability to manage a high-volume caseload efficiently
