Key Responsibilities

• Evaluate and treat patients in accordance with individualized physical therapy care plans
• Develop and implement evidence-based treatment programs to improve patient mobility, strength, and function
• Document patient progress and maintain accurate clinical records in compliance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and caregivers on therapeutic exercises and home management strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Jersey
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care setting
• Experience with geriatric patient populations
• Familiarity with electronic medical records (EMR) systems
