Job Overview
We are seeking a dedicated Physical Therapist to support patient-centered care within an interdisciplinary team environment. This role focuses on care planning, coordination, and delivery of rehabilitative therapy services to enhance patient independence and safety.

Key Responsibilities
Conduct physical therapy assessments in facility and home settings
Develop and participate in individualized care plans
Deliver and document therapy interventions
Provide active and passive exercises and rehabilitative treatments
Perform gait training and functional ADL training
Supervise Therapy Assistants and support program implementation
Educate staff, participants, and caregivers on therapeutic equipment
Requirements
Bachelor s degree in Physical Therapy
Valid Physical Therapy license or eligibility
